This is a professional position which involves activities in the continuing treatment, management, care and discharge of patients.  Duties require specialized skills, judgment in prescribing treatment and referrals as well as needed follow up.  Responsible for providing professional social work skills to staff, patients and their families--providing assessments, coordination, treatment planning, information and referral to community resources and other social work services.  Interacts with all hospital departments, employees, patients and families as well as outside healthcare and other human service agencies.  The focus is the patient's  social, financial and discharge planning needs in support of departmental and Medical Center quality and fiscal outcomes.  The goal is to aid patients and family in dealing with any medically related bio-psychological and emotional issues or concerns to assist patients/families maintain independence.  Cases are managed on a referral basis, both formal with a written order and informal as identified by the Social Workers themselves or from another member of the healthcare team.  Candidates must a Masters degree from an accredited college/university and possess (or be eligible for) a NYS LMSW license.  Previous experience will be a plus.
