Care Coordinator--Part Time
Reporting to CAP's Director of Care Management, this position provides direct care coordination for various populations in alignment with CAP network strategic goals and partnerships. The Care Coordinator collaborates with providers (inpatient and outpatient), hospitals, home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery. Applicant should have strong analytical, problem solving, critical thinking, organization, and decision making skills. The Care Coordinator must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with CAP network stakeholders and partners as assigned. Strong attention to detail with the ability to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity is a must.
Additional position requirements include current NYS driver's license, and the ability to transport self to customer base. Minimum three years of experience in a hospital or healthcare setting preferred. Strong written and verbal communication skills required. Previous experience in care transitions, population health, coding or health information management highly desirable.
